Numina

Numina is an attempt to create a pitch-tracker in Max/MSP for analysing the signal from a guitar (or ultimately any pitched source) and using the information to drive a synth or sampler device.

While an early attempt, further desired improvements include improved polyphony and perhaps MIDI output.
